- 5 grand prizes will be awarded, each consisting of a Walmart gift card in the amount of $1,000. Total approximate retail value of all Grand Prizes is $5,000.
- 750 first prizes will be awarded, each consisting of a Walmart gift card in the amount of $100. Total ARV of all First Prizes is $75,000.
- Entry Method:
- Make a purchase at Walmart and look for the survey code on your cash register receipt.
- Log onto www.survey.walmart.com within one week from the cash register receipt date.
- Complete the survey in its entirety and click the “Submit” button.
- Complete the entry form in its entirety and click “Continue”.
- Then, complete the check boxes and click “Submit” to receive one entry into the Sweepstakes.

- Deadline: 11:59:59pm CT on December 31, 2024
- Eligibility: Sweepstakes is open only to legal residents of the 50 United States, the District of Columbia or Puerto Rico, who are 18 years of age or older or the age of majority in their state of residence (which is 19 in Alabama, Nebraska, 21 in Mississippi, and 18 in all other states) as of the date of entry.
